Job Description Job Description Student Social Worker – Skilled Nursing Facility (SNF) Position Overview: AristaCare is seeking a motivated and compassionate Student Social Worker to join our interdisciplinary care team in a Skilled Nursing Facility (SNF) setting. This position is ideal for a social work student seeking supervised fieldwork or internship experience in long-term care, sub-acute, and rehabilitation services. The Student Social Worker will work under the supervision of the Social Services Director and/or Licensed Social Worker, gaining practical experience while supporting residents and families through psychosocial services and care planning. Position Details: Employment Type: PT Setting: Skilled Nursing Facility (SNF) Schedule: Between 7-5 Supervision Provided: Yes Preferred Requirements: actively in social work school Salary/Rate: unpaid internship Key Responsibilities: Assist with psychosocial assessments, care planning, and discharge coordination Provide emotional support and advocacy for residents and families Participate in interdisciplinary team meetings Complete documentation per facility and regulatory requirements Observe interventions related to mental health, coping, and social adjustment Requirements: Currently enrolled in an accredited BSW or MSW program Eligible for field placement or internship in healthcare/long-term care Strong communication, organizational, and interpersonal skills Interest in geriatrics, long-term care, or healthcare social work Basic computer skills; EMR experience a plus Powered by JazzHR hHHWmd0i34
